You may make an appointment with Dr. Kaplan by using her online scheduling system at www.appointmentquest.com; or by calling the office directly at 503.546.7663.

Generally Dr. Kaplan spends one hour with you at the initial appointment. If possible, please print the following forms and bring them, completed, with you to your appointment: Adult Intake Form, or the Child Intake Form, and the Financial Agreement Form. If you do not have access to a computer or printer, please arrive 20 minutes early for your appointment to complete the paperwork in the office.

Dr. Kaplan provides courtesy insurance billing. She is in network with ODS and Regence BCBS. It is always a good idea to check your insurance benefit before your initial visit. When you call your insurance company you may fill out this form to ensure you get all the correct information about your benefits.
